We provide support services to all aged NDIS participants across the Perth metropolitan area and surrounding regions, and up to 75 km from the Perth CBD. This ensures we can assist clients in both urban and regional locations. If you’re outside this area, please contact us to discuss options – we may still be able to help.
Assistance with Daily Living
We offer practical support to help individuals stay connected, independent, and engaged in their everyday lives. Our core services include assistance with daily living, which entails support with attending appointments, grocery shopping, running errands, and participating in community and social activities. We also provide companionship and assistance with planning and managing daily routines, budgeting and support in organising appointments.
Assistance with Social and Community Participation
We provide NDIS-aligned social and community participation supports to assist participants to actively engage in their community and build meaningful social connections. Supports are delivered in line with the participant’s NDIS plan and individual goals, and may include assistance to attend social, recreational, and community-based activities, appointments, and events. We support participants to develop confidence and independence to access the community in a safe and comforting process, which creates a sense of familiarity for the long-term future.
All supports are delivered in a strengths-based, person-centred approach, with a focus on promoting independence, choice and control, and fostering resilience, recovery, and overall wellbeing.
Psychosocial Recovery Coaching
Our Recovery Coaches provide participant-focused support that helps individuals with psychosocial or mental health needs work toward their personal recovery goals. This support involves building skills, resilience, and confidence to manage everyday challenges, improve wellbeing, and enhance independence. Recovery coaches work collaboratively with participants to set achievable goals, develop coping strategies, and access community resources and services. The support is tailored to each individual, empowering them to make informed decisions, strengthen social connections, and participate fully in home, community, and social life.
Support Coordination
Our Support Coordinators provide a participant-focused service that helps individuals understand, implement, and get the most out of their NDIS plan. Support coordinators work with participants to identify their needs, connect with suitable service providers, and coordinate multiple supports to ensure they work together effectively. They assist participants in navigating the NDIS system, building confidence and skills to manage their plan independently, and resolving any challenges that may arise. The aim is to empower participants to access the right services at the right time, achieve their goals, and participate fully in their community and daily life.
